   Liverpool play Burnley and Palace, so it is safe to say they'll get to 69 points.
Leicester finish with Spurs at home, while Chelsea finish with Aston Villa away.

Chelsea will beat Villa as if nothing else that'll be the dressed rehearsal for the CL final, Leicester need to avoid defeat at Stamford Bridge, if they win their in the CL, if they draw than they need to beat Spurs still on the final day.

Loose at Chelsea and they either need Chelsea to not win at Villa or Liverpool to slip up against one of Palace and Burnley, as well as beating Spurs.
