Privately owned aged-care facilities have taken over from religious, community-based and charitable organisations, and the sector needs a shake-up.The main providers of residential aged care used to be religious, community-based and charitable organisations; the quality of care may have varied but owners were not motivated by profit. During the past decade, however, the number of privately owned aged-care facilities has grown at twice the rate of those in the non-profit sector.

A recent report suggested that average profits in the industry rose 40 per cent last year, while the time spent caring for residents declined by 7 per cent.The federal government's Aged Care Financing Authority paints a more complex picture. Although profits are up, some facilities are doing better than others. Recent letters to the editor reporting medical negligence, neglect and inadequate personal care suggest that some facilities are prioritising profits over residents' quality of life.

Ten years ago, a Senate committee held an inquiry into the sector. Its report was critical of the accreditation standards of aged-care facilities, finding them too generalised to effectively measure care outcomes. Given that the accreditation process enables aged-care facilities to receive government funding, it should not be a rubber stamp.

Unfortunately, vague phrases such as adequate nourishment and hydration, effective continence management, optimum levels of mobility and sufficient staff continue to be used.More recently, a comprehensive list of quality of care indicators has been developed. This program is vital for encouraging continuous improvement and should be compulsory for all aged-care facilities; the government made participation voluntary.

A key to quality healthcare is a good staff-patient ratio. Without mandated ratios, many facilities operate with too few registered nurses and personal care attendants. Salaries are the main outgoings for an aged-care facility. Minimising staff numbers may maximise profits but it increases stress on those in charge.

Not surprisingly, there is a high rate of burnout among experienced nurses and managers, which lowers care standards even further. The rhetoric may be person-centred care, but the reality is somewhat different. On the morning shift, for example, residents are required to be toileted, showered, dressed, fed and medicated – all before 9am.

There is no requirement that these tasks be done thoughtfully; most staff are just too busy. Competent, honest and caring staff – managers, registered nurses, personal care attendants, as well as kitchen, reception and activities staff – are much more important than a nicely appointed bedroom or a lounge room with a coffee machine and grand piano.

Another problematic feature is the Aged Care Funding Instrument. This is used to pay subsidies based on each resident's level of need. It, too, is poorly worded and often serves the interests of the providers rather than residents.When a resident has been reclassified as requiring a higher level of care, staffing levels rarely change nor are extra services provided to the resident. The government recently introduced fines to curb a growing trend of incorrect, or false, claims for subsidies. Whether the fine of $10,800 for providers who repeatedly make false claims will act as a deterrent remains to be seen.

It is not only owners who may take advantage of residents. Some health care practitioners – GPs, dentists, podiatrists and so on – are also on the aged care gravy train. Recently, a dentistry service treated numerous residents at an aged care facility. Instead of charging a single "set-up" fee, each resident was charged this $90 fee on top of their bill.

Vocational providers are also claiming subsidies to offer aged-care courses despite some courses not meeting national standards. Age reporter Michael Bachelard has comprehensively illustrated how privatisation has turned vocational education "into a den of shonks and shysters".

Former ACCC chief Graeme Samuel describes this waste of taxpayers' money as the "inevitable consequence" of governments funding the private sector to deliver a public good.

Caring for older people with health issues such as dementia and incontinence is a demanding job that requires specific expertise. An "accredited" fast-tracked course does not equip graduates to work competently with older people, particularly those from culturally and linguistically diverse backgrounds and the gay and lesbian community.

Working with older people requires staff who, at the bare minimum, speak English fluently and are able to read and update care plans. Ideally, facilities would have staff who are kind and have a genuine interest in older people. Unfortunately, kindness cannot be taught or bought.

To ensure older people living in aged-care facilities have the best possible quality of life, relatives need to become more involved in their care. It is not enough to simply pay the fees and hope for the best.My mother spent her last five years in an expensive facility in which the bond (a de facto interest-free loan) was $623,000. I visited her most days, so became acutely aware of the stresses on staff and the corners that are cut to maintain high profitability. In 2012, relatives at Mum's facility were concerned about inadequate care. We documented incidents of negligence, incompetence, staff not telling the truth, bullying and racial vilification.

We also reported numerous thefts, though this was difficult to prove because victims were invariably people with dementia.

Fortunately, the owner responded positively to our list of our grievances. Most importantly, he replaced the manager. Good managers are the linchpins of a quality facility.

Towards the end of Mum's life, only the most experienced staff were able to provide adequate care. Those with less than four months training did not have the required clinical skills. For two months, I sat at my mother's bedside to protect her from inflexible routines and policies. I ensured she slept as long as she needed, and ate when (and if) she wanted.

After a week or so at her bedside, the manager asked me to stop interfering. I refused to budge, because I did not have confidence that staff could do their jobs.

The aged-care sector needs a shake-up. The key players have competing interests: residents and their relatives want high-quality care while owners focus on profitability. The government must increase regulation because the care of vulnerable people is too important to be left to the free market.ged-care facilities require meaningful accreditation standards, compulsory quality of care indicators, a more rigorous Aged Care Funding Instrument, better training of staff and mandated staff ratios. We need to ensure older Australians receive the quality of care they deserve.

Aged care is presented to the Public with beautiful surrounds and the elderly communicating with company.     Such things as entertainment , bus trips and everything one would expect and hope to see.

   Don't be fooled , It is what you will never see is where the horrors exist and is nothing more than the hordes , herded into groups with failed minds.    Time consuming times of trying to feed unco-operative unfortunates, ( I suggest too many don't get fed much) Bathe & change the incontinent , Give out medications in very limited time ( most of which goes down a sink when time runs out )
Staff are buried in paper work , The No 1 priority may I add .
The Patients are a necessary evil .
They are Horror Camps I can assure you AND will be informed in advance if anyone of authority is likely to pay a visit.

No sympathy for a society that wants the government to stay out of our lives, you can not have it both ways.

I will state right here and now, while watching that four corners program which is the only reason sco-mo got off his backside on the sunday and announced a royal commission, that if any person or organisation treated my mother as they did, when described by those workers, that company would be out of business so quick they would be glad to leave Australia and never come back.

If the man from the industry body is anything to go by then there is no hope, fancy sitting there and saying in public that $6 a day for three meals is adequate. fiddlesticks

These are not the only places that have been exposed, the child welfare dept, foster care,indigenous youngsters slipping through the cracks and there would be others but at least he's been made aware where he's now in a position to DO something, don't knock him, you may find this man a surprise packet, life is full of them.

Sad to say that my grandmother was put into a home by her children who immediately sold her extremely large block of land in suburban Brisbane (around 4,500 sqm from memory) to access the cash. All successful people mostly in the medical field and could easily have afforded to pay for a full time carer in the house she’d lived in for 60+ years.

I have almost nothing to do with that side of the family including my father. Would see one or two of them once a year at most. So yes not my place to say anything but for a group of quite well-off people to choose a nursing home over what would have been maybe $25k each per year for a decade or so left a sour taste in my mouth.

You are all correct,   The situation covers such wide areas , From beginning in minimum care areas, to the gradual decline to final days .
The journey between is an horrific experience for far too many and despite good intentions of so many of the limited staff ,     It's not good enough.

Demented people especially are so difficult to care for ,   Over time these people disappear from Public view in the main and with zero quality of life it is a rough road to final peace.

The world would do most who have reached these helpless, mindless stages , a favour, by doing ( you know what ).
Instead in many cases with our stupid laws , the Medico's ( in fear ) are more likely to insert a feeding tube ( PEG ) through the abdominal wall to ensure more suffering.
They can hold 1000 Royal Commissions about MONEY and costs but it won't change the sad , end of life experiences.

One would be surprised at how far this goes with some people.....

To cut a long story short * Encountered someone through work taking care of their father's last few weeks and they even went as far as going to the kitchen and telling the Chef what was what with food for her father and good on her it was needed .... although he did eventually say to her ...please let me go Wink

Anyway very good move ... best thing done in a while by a Government  (in this Country)

It should be mandatory to have cameras in the rooms.

I find it despicable what some of these "carers" do to our parents.

We should be so much more suspicious and not be afraid to question and query.

When my dad was in a home, i had a chat with a Philippina carer. She said when it came time she would be going back home to look after her mother in her home. She didnt like how it was taken care of here.

There must have been something in that.

I know that i feel uncomfortable about his care, now, after the 4 corners piece.

I saw an 85 year old man this morning , covered in bruises . ( slightest bump to him being on blood thinners , causes these haematomas.) but one has to question if he has been physically abused.
He is resistive to everything.   Showering ,shaving , eating etc.
An impossible person.
Argumentative, confused . Even his own children stay away as they have their life to live and aren't appreciated for their kind efforts .
This man is a burden on the taxpayer , won't go on his prescribed morning run , claims his joints are metal to metal and his walker has a flat tyre .   Who knows what medication he takes as that is one of his last secrets .
   I did some basic investigations ,   as he lives alone .
Chit = I just realised ,      It's me I'm talking about.

Last night was an indictment on our society!

So before the last inquiry, we had the government doing 3000 visits to respond to complaints, but last year after over 4,000 complaints we had 50 occasions where the officers from the authority actually visit the complex.

Then as if that was not enough, we see that 98% of the authorised facilities, have scored a 100 out of 100 for compliance.

VOYAGER:-     I didn't watch the shows.     To much is codswallop.
There will always be the occasional beast working in these areas.
Most are truly caring' however , so overloaded that there is rarely a minute extra to give special time an individual.

It is an impossible task to provide the level of care so many need.
We would never allow our pets to suffer & be subject to the slow grinding suffering in a shocking existence on the way to end of life.

There are areas where quality of life is acceptable but that gradually diminishes as nature does it's part in reducing the individuals ability to cope without total care.    It is something we justifiably , all fear.

We need to get PETA on the job - that kind of treatment would never be tolerated on a Live Exports Ship - those ships are cleaner than some of those homes too!Dead

In saying that, this wouldn't have been a typical Cut Corners beat up would it? ... I didn't watch it, but did they actually interview any of the alleged abusers, or get the other side of the story? ... plenty of "we invited ..." and "... had no comment" I would imagine.

Most are truly caring' however , so overloaded that there is rarely a minute extra to give special time an individual.

It is an impossible task to provide the level of care so many need.
We would never allow our pets to suffer & be subject to the slow grinding suffering in a shocking existence on the way to end of life.

There are areas where quality of life is acceptable but that gradually diminishes as nature does it's part in reducing the individuals ability to cope without total care.    It is something we justifiably , all fear.

Yep ... and it puts the need to plan for your retirement into perspective, doesn't it - that's why I won't be relying on anyone but me! 

I won't be retiring until I have enough in the bank to ensure that I can call in a favour from old mate Peter Dutton, and have an endless line up of young nubile French Au Pairs, who will be able to care for me in my own home, in the manner to which I am accustomed, right up until that last breath ... and they won't need 6 languages, I don't mind if they speak English or not, so long as they are blonde, and they can reach high to get stuff from the top shelf of the cupboard (with apologies to Benny Hill)Wink.
